Dr. Lukas Pytlik

TMJ & TMD Treatment in La Jolla

Jaw pain, clicking or popping, mornings that start with a tight, aching face, headaches that never quite resolve: temporomandibular disorders (TMD) wear people down precisely because they touch everything: eating, speaking, sleeping, concentrating.

The jaw joint doesn't operate alone. It works in constant partnership with the muscles that move it and the teeth that guide it. Dr. Pytlik treats TMD from that whole-system perspective, identifying whether the joint, the muscles, the bite, or (commonly) all three are driving your symptoms, and treating the cause rather than muting the symptom.

Our Approach

Diagnosis Before Treatment

TMD has multiple distinct causes that require different treatment. Dr. Pytlik's evaluation examines the joint, the muscles, your bite relationships, and wear patterns on the teeth (which record your jaw's history like rings in a tree) before any treatment is proposed. He also takes the time to walk you through how your TMJ problem developed and whether it stems from injury or from an underlying pathology. That distinction matters: when you understand the cause, you can be confident you are not receiving treatment you don't actually need.

Calibrated Occlusal Appliances

Far beyond a boil-and-bite guard, a precisely designed occlusal appliance repositions the jaw to decompress the joint and gives strained muscles a stable, neutral position to rest in. Design and calibration are where specialist training shows.

Neuromodulator (Botox) Therapy

Where overactive jaw muscles are the pain generator, targeted neuromodulator therapy reduces the excessive clenching force at its source, an approach Dr. Pytlik already uses successfully in his bruxism treatment program.

Bite Correction Where It's the Cause

When the way your teeth meet is feeding the dysfunction, Dr. Pytlik can refine the bite itself, from conservative occlusal adjustment to restorative correction, drawing on the same expertise behind his advanced occlusal disease management program.

Signs You Should Be Evaluated

Jaw pain or fatigue

Especially in the morning, or after chewing and speaking.

Clicking, popping, or grating sounds

Joint noises signal the disc and joint aren't moving in harmony.

Limited opening or locking

A jaw that catches, deviates, or won't open fully.

Frequent headaches or ear symptoms

Temple headaches, ear pain, or fullness without an ear infection.

Worn, flattened, or chipped teeth

Visible evidence that grinding and bite stress are at work.

Common Questions

What's the difference between TMJ and TMD?

TMJ is the temporomandibular joint itself: the hinge connecting your jaw to your skull. TMD (temporomandibular disorder) is the family of problems affecting that joint and its muscles: pain, clicking, locking, restricted opening, and referred symptoms like headaches and ear pain. People commonly say 'I have TMJ,' but TMD is the condition being treated.

Can a bite problem really cause my headaches?

Frequently, yes. When the bite forces the jaw muscles to work in a strained position, or clenching and grinding overload them, the resulting muscle tension commonly refers pain to the temples, behind the eyes, and into the neck. Patients are often surprised that tension-type headaches improve when the underlying jaw dysfunction is treated.

How does treatment work without surgery?

The large majority of TMD improves with conservative care. Dr. Pytlik uses precisely calibrated occlusal appliances to decompress the joint and quiet strained muscles, targeted neuromodulator (Botox) therapy where muscle overactivity drives the pain, and bite refinement where the way the teeth meet is a contributing cause. Surgery is a last resort reserved for specific joint pathology, and referral is made when it is genuinely indicated.

Why see a prosthodontist for TMJ problems?

Because the joint, the muscles, and the bite operate as one system. Prosthodontists are the dental specialty trained most deeply in occlusion: how the teeth meet and how those forces travel through the jaw system. Treating the joint without understanding the bite treats the symptom and leaves the cause.
